New Patient Registration
If you wish to register with the practice please telephone or call in and one of the reception staff will be able to help.
The receptionist will explain the procedure to you, provide you with the relevant paperwork, and book an appointment for you to see one of our nurses or our healthcare assistant.
Before your appointment, you will be asked to fill out some forms about your current and previous health conditions, the medications you take and the details of your previous practice.
You will also be required to provide proof of your identity and address, in the form of photographic & a houshold utility bill or bank statement. You are also required to bring proof of any medications you are currently taking to the appointment.

Due to the increasing pressures on Primary Care we are unfortunately unable to register patients who live outside of our catchment area. Should you move outside of our catchment area you will be asked to register with a GP nearer to where you live.
It can take several weeks for us to receive your medical records from your previous practice, so the more detail you can give us the better.
Once your registration is complete you will be informed of your named GP. This however does not mean you can only see this particular Doctor. You have the right to express a preference to see a particular practitioner, and whilst staff will always do their best to accomodate patient preferences, this might not always be possible. We will always offer alternatives, and compromises may need to be reached on occasions.
Medical treatment is available from the date of registration.
